Total omega-3 measures the combined percentage of all omega-3 fatty acids (EPA, DHA, ALA, and others) in red blood cell membranes, reflecting long-term dietary intake and supplementation over the prior 3–4 months. Higher omega-3 status is associated with reduced cardiovascular mortality, lower inflammation, better mood regulation, and protection against age-related cognitive decline. The Omega-3 Index — specifically the sum of EPA and DHA — is the most clinically validated fraction, with a target above 8% for cardiovascular protection.
