About White Blood Cell (Urine)

White blood cells in the urine (pyuria) confirmed by microscopy — typically defined as more than 5 WBCs per high-power field — indicates inflammation or infection within the urinary tract or kidneys. In combination with bacteria on microscopy and a positive nitrite dipstick, pyuria strongly supports a urinary tract infection. Sterile pyuria (WBCs present without bacteria) can indicate kidney tuberculosis, interstitial nephritis, chlamydial infection, or an inadequately treated UTI.

Elevated WBCs above 5/HPF confirm pyuria from bacterial UTI, pyelonephritis, or interstitial nephritis. Very high counts with bacteria indicate active infection.
