It is a more complete risk marker than LDL alone because it includes cholesterol carried by triglyceride-rich particles, which are particularly harmful in people with diabetes or metabolic syndrome. Major guidelines now recommend Non-HDL as the primary treatment target over LDL-C in high-risk individuals.

Standard target is below 130 mg/dL. Functional practitioners target below 120 mg/dL. More strongly correlates with cardiovascular events than LDL.
