Total T3 measures both protein-bound and free triiodothyronine, the most metabolically active thyroid hormone. It includes T3 bound to transport proteins, making it susceptible to changes in binding protein levels from pregnancy, estrogen therapy, or liver disease. Free T3 is generally preferred for clinical decisions as it reflects biologically available hormone.

High total T3 indicates hyperthyroidism or excess thyroid hormone. Low total T3 occurs with hypothyroidism, nonthyroidal illness syndrome, or conditions that reduce binding proteins.
Standard range is 80-200 ng/dL. Free T3 is generally preferred over total T3 because it's unaffected by binding protein changes from pregnancy, estrogen therapy, or liver disease.
