Apolipoprotein A-I (ApoA-I) is the primary structural protein of HDL cholesterol, essential for reverse cholesterol transport — the process of removing excess cholesterol from your arteries back to the liver. Your ApoA-I level reflects your functional capacity for cardiovascular protection. It is a stronger predictor of cardiovascular risk than HDL cholesterol alone.

High ApoA-I is cardioprotective, reflecting robust HDL particle numbers. Low ApoA-I indicates insufficient HDL-mediated cholesterol removal from arteries.
Standard range is 100-175 mg/dL. Functional target is above 140 mg/dL for men and above 150 mg/dL for women. The ApoB/ApoA-I ratio below 0.6 is optimal.
