IgM is the first antibody produced during an acute immune response — highly efficient at agglutinating pathogens and activating complement. Elevated IgM signals acute or recent infection, while persistent elevation can indicate Waldenstrom macroglobulinemia. IgM in a newborn above 20 mg/dL indicates in-utero infection.

High IgM indicates acute infection, primary biliary cholangitis, or Waldenstrom macroglobulinemia. Low IgM is seen in combined immunodeficiency and nephrotic syndrome.
Standard range is 40-230 mg/dL. Optimal is 50-200 mg/dL. IgM in a newborn above 20 mg/dL indicates in-utero infection.
